Separate conduct from performance, test whether the problem is premature, and identify expectation, process, provider, person, or solution failure.
Immediate gate
- No suspected privacy or security breach
- No deliberate concealment or falsified work
- No unexplained disappearance or prohibited access behavior
- If conduct is suspected, access and evidence have been secured and professional advice obtained where needed
Timing and fairness
- The person has had enough normal ramp time to judge the affected workflow
- The review accounts for migrations, staff departures, or other unusual disruption
- Expected output and quality were written
- The person received a usable procedure
- Decision rights and escalation rules were clear
- Specific corrective feedback was given more than once
- A manager had time and responsibility to support the role
Cause test
- We answered: “Would a different person succeed in this same setup?”
- Current scope was compared with the original agreement
- Work volume was compared with contracted hours and capacity
- Provider staffing, availability, training, and management claims were checked
- Repeated tasks were assessed for process redesign or automation
